Dear Mr. Rylee:

Enclosed is the analysis you and other breast implant manufacturers requested in our April 14, 1992 teleconference. Titled "Detail of Registry Operations and Costs for a Voluntary Retrospective Breast Implant Registry", it contains recommended procedures as well as sample costs for a retrospective registry.

In addition to preparing this analysis, I attended the FDA meeting of April 27, 1992, apparently representing the only input the FDA has received from an epidemiologist with registry experience. In opposition to the complex registry described. previously by the FDA, during this meeting Dr. Ralph Cook and I urged a much more streamlined and cost-efficient design. The FDA agreed to this approach.

I suspect that you are unaware that I have provided many hours of

consultation to Dow Corning regarding breast implant safety issues over the last several years without compensation. While I am pleased with my contribution to the significant progress that has boon achieved in designing a rational approach to a retrospective registry, my effotrts over the last 30 days have been devoted almost exclusively to these issues. I am asking each of the manufacturers for support of these activities. From Dow Corning, I

request $5,000 in consultation fees. My taxpayer ID is 536-52-3865.

While I believe that my efforts have significantly reduced unnecessary expenses associated with a retrospective implant registry, clearly this post hoc request carries no legal obligation. Thank you for your consideration.
